evaluation of parameters influencing bare descemet membrane achievement during deep anterior lamellar keratoplasty in keratoconus

چکیده
purpose: to assess parameters that had a capability of influencing the rate of big bubble formation during deep anterior lamellar keratoplasty (dalk) in keratoconus. methods: a total of 290 consecutive eyes from 257 patients with keratoconus that underwent dalk using the big-bubble technique were enrolled. the influence of patient age and sex, family history of keratoconus, history of contact lens wear or vernal keratoconjunctivitis, the presence of vogt’s striae or superficial opacities, keratometric readings, central and peripheral corneal thickness, anterior chamber depth, vitreous length, and trephination size on the rate of a bare dm achievement was investigated. results: dalk was performed in 289 of 290 eyes and a bare dm was successfully achieved in 229 (79.2%) eyes. the recipient sex and trephination size had a significant association with the success rate of big-bubble formation. the female sex decreased the odds of bare dm achievement by 0.44 times (p=0.02). for each 0.1-mm increase in the trephination size, the odds of the successful big-bubble formation increased by 1.36 times (p=0.03). other factors failed to demonstrate any significant association with the rate of successful bare dm achievement. conclusion: the rate of successful big-bubble formation was 79.2% in keratoconus. among different factors, female sex predicted a less likelihood of big bubble formation while large trephination size was significantly associated with an increase in the chance of bare dm achievement.

Purpose: To determine the etiologic factors on rejection and failure rates and ocular surface changes after corneal transplantation by either DALK or PKP, in cases of keratoconus (KCN). Methods: In this cross-sectional study, considering the existing data, patients with keratoconus referring to Labbafinejad Medical Center from 2006 to 2016, who underwent corneal transplantation, were enrolled a...

PURPOSE A single-center, cross-sectional study was designed to assess and compare objective and subjective quality of vision of patients intervened with penetrating keratoplasty (PK), deep anterior lamellar keratoplasty (DALK) and Descemet membrane endothelial keratoplasty (DMEK). METHODS Forty-six patients previously intervened with PK (22 eyes), DALK (7 eyes) and DMEK (17 eyes) were recruit...
متن کاملAnterior Lamellar Keratoplasty in Keratoconus
Keratoconus (KC) is a progressive, bilateral, but usually asymmetrical ecstatic corneal disease, characterized by progressive corneal protrusion and thinning, that leads to corneal surface distortion. The reported incidence in the general population varies from 1.3 to 25 per 100,000 per year across different populations, with a prevalence of 8.8–229 per 100,000. Onset typically is at puberty wi...

RATIONALE To report a case of late spontaneous reattachment of Descemet membrane (DM) detachment after deep anterior lamellar keratoplasty (DALK). PATIENT CONCERNS A 50-year-old woman with stromal corneal scarring underwent a DALK with the big bubble technique. The DM detachment and double anterior chamber were observed on the first day after operation. DIAGNOSES Descemet membrane detachmen...
متن کاملDouble-bubble technique to facilitate Descemet membrane exposure in deep anterior lamellar keratoplasty.
Safe and efficient exposure of Descemet membrane is the key to success in deep anterior lamellar keratoplasty. Although widely used, the big-bubble technique has the drawback of difficulty in maintaining appropriate needle insertion depth in the corneal stroma, resulting in injected air sometimes escaping to the peripheral cornea without separation of Descemet membrane. We describe a variation ...
